New Member Enrollment Application (NMA)

Version 1.05

  • Put last name first, made space larger on name lines, added “surname” and “family name” to accommodate cultural differences in name patterns. See “Naming Customs “ PDF on Treasurers’ page of: https://frrsma.com/treasurers/

Version 1.06

  • Added back the reference to 2%, and described it as applicable to pretty much everyone on the portion of their annual wages over $30k. See the “2% on $30,000” information on the Treasurers’ page of: https://frrsma.com/treasurers/
  • Moved version notes to our website (here) and noted it with the version number at the top of the title page on the form.
  • Added page numbers as “1 of 8”.

Version 1.07

  • Left the 2% box unchecked to aid treasurers using this form as a place to note if this person is earning more than $30k annually, and if the 2% should be set up in their payroll program.

Version 1.08

  • Changed the FRRS address to 101 Munson Street.

Version 1.09

  • Rearranged the Applicant’s Signature page so that is was clear that they were signing for all three sections above and not just the SS-1945 (which immediately precedes the signature page).
